Clients with automatic billing arrangements: no charge if cancellation
is of the emergency nature and scheduling of the new appointment occurs within 48 hours
of the missed appointment; however, if the second appointment is missed,
$150.00 will be charged. All others, charge of $75.00 immediately after
appointment is missed. |

Have your preferred time of consultation and two alternatives. Have your
credit card ready -- Visa, MasterCard. You will be charged $40.00 upon
setting the appointment and balance -- if any -- upon completing the
consultation. Telephone connection fee will be added not to exceed
$6.00 per hour in ten-minute increments for US (incl. Alaska and Hawaii)
telephone numbers. International calls will be billed actual costs.
